APPLICATION CHECKLIST

 

Please carefully review the following application checklist. NHS must receive all applicable items/information listed below before they can begin processing your application. Applicants will be notified by NHS of missing items but their applications will not be processed until all missing items are submitted.

For Applicant(s) only:

  • Listed Auto License Plate Numbers and State on application
  • Attached tenant leases or 3 most recent rent receipts for rental units if there are 2 or more units in the property. If the units are vacant or are occupied by family not paying rent, please attach a NOTARIZED LETTER of explanation.
  • Attached copy of Illinois Driver’s License OR copy of State of Illinois Identification card for all property owners
  • Attached proof of current homeowners/property insurance (i.e., copy of homeowners insurance declaration page)
  • Attached a NOTARIZED LETTER from any person(s) on title NOT living in the home indicating that they approve of grant’s terms, conditions, and use to make home improvements (if applicable)
  • Attached proof of ownership (i.e., copy of Recorded Deed and/or a copy of the most recent mortgage statement, if there is a mortgage on the home)
  • List the name, date of birth, and Social Security number of all household members on application. NOTE: For 2-4 unit FAMILY BUILDINGS, the Applicant’s household is considered by the City of Chicago to include all family or friends living in a separate unit(s) that do not pay rent
  • Attached copies of Driver’s license or State Identification for all household member 18 years of age or older
  • Attached copies of 2018 and 2019 Federal Tax Returns including all Schedules and W-2 Statements for all household members 18 years of age or older. If you have not filed year 2019 Federal Income Tax Return, then please submit your 2017 and 2018 Tax Returns with all schedules and W-2s
  • Attached SIGNED AND NOTARIZED copies of current Profit and Loss Statement for all household members with Self Employed Business
  • Attached a NOTARIZED LETTER signed by any household member 18 years of age or older that does not file Federal Income Tax Returns due to being on a fixed income (i.e., on Social Security or Pension income)
  • Attached 3 most recent pay stubs (showing year-to-date earnings) for all household members 18 years of age or older. If a household member is receiving unemployment income, the most recent unemployment documents are required
  • Attached copies of 2020 Social Security award letter showing MONTHLY amount awarded for any household members that receive Social Security income
  • Attached copies of 2020 SSI award letter or statement showing amount awarded for any household members that receive SSI income
  • Attached copies of 2020 Pension award letter showing MONTHLY amount awarded for any household members that receive Pension income
  • Attached copies of current Unemployment Statement for any household members that receive Unemployment income (online printouts will not be accepted)
  • Attached public assistance award letters for all household members 18 years of age or older (if applicable; exclude SNAP/Link benefits)
  • Attached NOTARIZED LETTER of explanation of no income signed by any household members 18 years of age or older who are not employed (must include last date of employment) or are full time students
  • Signed and dated the application

For 2-4 Unit Properties Only:

  • Completed City of Chicago Tenant Self Certification of Annual Income Form for each tenant-occupied unit
  • Tenant Self Certification of Annual Income form(s) signed and dated by each tenant household member
  • Copy of current lease or 3 most recent rental receipts for each tenant-occupied unit
  • Copy of Illinois Driver’s License or State of Illinois Identification Card or most recent utility bill showing name and address of primary renter for each unit (should match lease or rental receipt)
