Required Application Materials for Target Block, MMRP Target Areas, and 606 Trail NIP Grants

Once your preliminary Target Block or MMRP Target Areas Grant application is reviewed, you will be invited to submit the remaining documents needed to complete your application. All application materials must be uploaded online and should be submitted at one time. The grant application and all supporting documentation for the 606 Bloomingdale Trail NIP grant may either be mailed or submitted in-person to the Central NHS office (1279 N. Milwaukee Avenue, Suite 400, Chicago, IL 60622). Required application materials include:

  • A copy of the property owner’s Illinois Driver’s License or State ID Card.
  • Two most recent years (2016 & 2017) of Federal Income Tax Returns with all of the schedules attached. Please submit Federal Income Tax Returns for any and all household members 18 years of age and older. Illinois State Tax Returns are NOT required or needed.
  • Two most recent years (2016 & 2017) W-2 statements for any and all household members 18 years of age and older.
  • Three most recent (past 30 days) paystubs for any and all household members 18 years of age or older.
  • 2018 Social Security award letter showing the MONTHLY amount awarded for any and all household members 18 years of age and older.
  • 2018 Pension Statement showing the MONTHLY amount awarded for any and all household members 18 years of age or older that receive Pension income.
  • Most recent mortgage statement OR copy of RECORDED deed if you no longer have a mortgage.
  • Current homeowners insurance declaration page.
  • Lease agreement for rental units in the property (2-4 units) OR copies of 3 most recent rental receipts if you do not have a lease. Owners must submit a notarized letter if they do not receive rental income for a unit (i.e., family building, not tenants, etc.).
  • If you do not have an income, please provide a notarized letter stating that you do not receive any income, unemployment income, and/or are not working. This applies to any household members 18 years or older. Letter must be signed by the household member declaring no income.
  • Notarized letter if you do not file federal income tax returns due to being on a fixed income (i.e., on Social Security and/or pension). This applies to any and all household members 18 years of age or older. Letter must be signed by the household member declaring they do not need to file tax returns.
